It's just a simple spawn-able model like any other you can animate. Are you struggling to understand how to change skins &/or body groups maybe?

I otherwise don't know what you mean by 'how does it works?'

To me SFM and blender are the most simplest 3D softwares out there but it will always be complicated. Just a type of complicated I can work with.

I am just learning Sfm on the weekends or whenever I have the time. it is sorta like Blender or learning something complicated.

Yeah I figured you ment that. But anyway you can make the maps for the tf2 game itself and gmod. You can make the map ridiculously massive. Not GTA 5 big but pretty big if you're famillier with the gmod map "bigcity" here: https://www.48675488.xyz/sharedfiles/filedetails/?id=671949574

There a lots of guides and tutorials out there to help get to know about hammer of course. Best start with basic maps and work your way up.

It's where you go to make the maps. Also you can apply game logics to it to tell the map to behave like a gameplay say TF2 "capture the flag" or "MVM" but the game logics are a lot more complex. Making maps is quite easy once you get use to the hot keys and stuff but when converting the files to work with SFM is where the conflicts attack U. Hammer lists down every conflict but you have to fix it more or less on your own.

Hammer comes with SFM. to access it you go launch SFM from steam (not desktop shortcut) and click on "launch sdk". When SDK comes up is should show the option to launch hammer.

Hammer is even buggyier than SFM, as it is hammer who bombards me with the conflicts.

I would really like to put maps on the workshop but I can't. I've been making maps but when I try to convert it to work on SFM there's almost always a HDR issue and on top of that even when I take an exsisting map to make little changes hammer bombards me with dozens of conflicts that I have no idea how to fix. So unless you know someone who'd help me out with all that I'm at rock bottom.
